License Verifications. Licensee information is available at the Board's website at Alphabetical Listing of Oregon Optometric Physicians.The Oregon Board of Optometry will provide written verification of a licensee's license status with the Board upon written request. There is a fee of $20 per license verified. Payment must be received before the verification is processed.
Licensee Lists. Computer printout lists or disks of Oregon licensed optometrists are available upon written request. There is a fee of $25 per hard copy or floppy disk of Oregon licensed optometrists. Payment must be received before the list is processed.
